A critical part of the performance evaluation process is to engage staff in self-evaluation. This template provides a worksheet to capture information about a practitioner-level staff member’s perception of their annual performance. When filing the performance dimensions to be assessed, include:

  • Responsibilities from the job description.
  • Assigned goals from the strategic plan.
  • Needed skills and behaviors for the defined job/role.

Each job/role may need its own template.
