A critical part of the performance evaluation process is to engage staff in self-evaluation. This template provides a worksheet to capture information about a management-level staff member’s perception of their annual performance. When filing the performance dimensions to be assessed, include:
- Responsibilities from the job description.
- Assigned goals from the strategic plan.
- Needed skills and behaviors for the defined job/role.
