This questionnaire provides a list of questions to gain input from your employees on the collaborative behaviors and actions that are occurring in your organization. It can be used in two ways:

  1. Use the questionnaire as a KPI metric to track improvement. Send this questionnaire to your focus group members to gain insight into the collaboration issues from your employees point of view. Distribute prior to launching any initiatives to improve collaboration and six months after the launch.
  2. Use these questions to help structure an in person focus group discussion on interdepartmental collaboration.
