When employees violate policies, they must be reprimanded accordingly. Depending on the severity of the violation, the penalty could include anything from a verbal warning to termination. Written warnings are most often used as a second step in the process, after a verbal warning.

This template includes:

  • Fill-in-the-blanks for the policy name, the number of instances, and the level of violation.
  • Space to expand on the nature of the policy violation.
  • The actions that will be taken as a result of the violation.
  • Required signatures.

Use this template to draft an official policy violation letter once guilt of transgression is proven.
