While the form is not as important as the discussion, you must still capture the discussion in writing to keep track of the decisions that were made and who is responsible for what tasks.

Specifically, this template will:

  • Document employee strengths and opportunities.
  • Document employee development goals and a timeline for meeting those goals.
  • Document the methods the employee will use to achieve the goals.
  • Include details on the type of support that the manager will provide.
  • Allow for development progress updates.

This template documents the decisions made after discussing 360 degree feedback with a manager or facilitator.
